Transcripts Requests
To request a transcript, please fax the high school office at: (309) 462-2492. Please include the following information and your signature on the fax.
1. Full name and maiden name (if applicable)
2. Graduation year
3. Date of birth
4. Name and address of where to send transcript
5. Signature
There is a $1.00 fee for transcripts. This can be mailed to AMHS at:
600 W. Martin St.
